That might depend much on where she lives. As mentioned in the FDA literature, clearance of the drug increases during pregnancy, and actually a doctor might prescribe a higher dose to make up for the short fall. Her current drug source might not be that pure, and there are many pitfalls for both mother and child if her doctor is not informed well in advance to make clinical adjustments. Other issues related to drug abuse like HIV and other infections need to be looked at. I will not repeat the fda information here, it is straight forward in the above article."

Laureen K replied: "There are multiple issues with your friend being addicted to methadone. First, she needs to be upfront with her doctor. If a woman goes through pregnancy addicted to drugs and is not forthcoming, the hospital most likely will file with department of social services and the child could end up in foster care. If on the other hand, she tells her doctor, she can get treatment for the addiction and show the she is being somewhat responsible by getting treatment. Also, if the hospital knows ahead of time about her drug use, her baby can be monitored closely for withdrawal and given medications to help with symptoms If she does not tell them, her baby can end up in a hellish withdrawal without getting treatment. In terms of effects on the baby, when the baby is born, he/she will go through withdrawal, which can be pretty awful! Crying inconsolably, jitters, throwing up, fevers. The baby may need to be hospitalized for care related to withdrawal. Please urge your friend to get help."

Your dose is lower than most pregnant women, but the dosage required varies and is based on whether you are experiencing cravings or withdrawal. As your pregnancy progresses, you may need a higher dose because of the increase in blood volume as you and the baby grow. Let your doctor know if you are symptomatic. Undergoing methadone treatment while pregnant will not cause birth defects for your baby, but some infants may go through withdrawal after birth. Methadone and subutex and pregnancy? I know that none of these are the best thing to take during pregnancy, does anyone know which is better to take during pregnancy? 5-10ml methadone every other day up to 2mg subutex every other day? (very small amounts) Its better to take than be clucking and i know that the amount taken will not reach the baby!! no im not an addict!!! and i would never give my child up for addoption i stopped smoking h before i was even pregnant, have been completely clean from everything but sometimes you feel you need it (not h) but meth oe subs i just want to no which is safer as i no that up to 80ml of methadone will not pass to the baby which i would never use that much i just want to no about subutex!! im on a meth script but ive been on a subutex script before and felt better in my self but i dont no about in pregnancy!! im actually not even taken all the meth i get as i feel he has given me too much!!

Ruth B replied: "Methadone is a very safe drug to be on while you're pregnant. Don't let anyone here make you feel badly about this choice. Continuing to use opiates such as vicodins can cause holes to be in your unborn child's heart. (Not good) Going cold turkey can cause miscarriage. (Not good) I would suggest you enter a methadone treatment program immediately. They are able to monitor you and your dose as well as test your urine for illicit substances and coordinate with your doctor(s). You should also be able to see the methadone clinic physician every month or more often if needed. I'm not that certain about subutex/suboxone. I have friends that had children on methadone and their children were born with very few if any symptoms of withdrawal. Very healthy children. Good luck and God bless! That's why I suggested that you go to a clinic as they can monitor you. A prescription with a 30 day supply leaves too much room for error. You may end up taking too little or most importantly too much! Not to mention that you're not getting the support that you need so badly right now. If you're still taking the methadone prescription and you switch to suboxone I believe that you will go into immediate withdrawal. If you haven't taken anything (including methadone) for the past 2 or 3 days, you may be able to take subutex/suboxone. Though I would absolutely bring your ob/gyn and prescribing doctor into the picture and ask them the best route. Get the experts involved. There's nothing wrong with getting treatment if you're having strong cravings."

Mz.Rahi replied: "During Pregnancy- Safe Methadone Dosage- For chronic pain patients (Yes people, methdone is a COMMONLY USED and VERY SAFE medication for people with neurogenic chronic pain - NOTHING to do with drug abuse) up to 40 mg./ day at 10mg/ q.4 intervals For substance abuse I believe it is up to 120 mgs. I don't know about the subutex, but with either med you will need monitoring by a perinatologist just to be safe, although they are rarely needed."

methadone and early pregnancy? First of all it is not me that is pregnant but my question is my freind is 7 weeks pregnant and she had been on 10mg of methadone a day due to back pain but is not prescribed by a physician. Do you think that her baby will be ok? If she tells her mid-wife that she is dependent on this will she tell authorities or not? I dont know much about the withdrawal of this or the side effects during pregnancy so should she reduce the doses until she is off of it or should she go cold turkey? Thanks in advance

If she has back pain, there are far safer drugs to take during pregnancy. Methadone is usually abused by people who were taking lower pain killers. It's a horrible addiction, speaking from experience with my fiance. His ex-wife was a Methadone addict with the child she just had. Luckily the baby has no problems as of this moment...but not everyone is that lucky. Please advise her to speak to the doctor about it...they can offer her help. westin replied: "Methadone will not 'kill' her baby. I have worked in a methadone treatment facility for 10 years and seen lots of happy healthy babies born to mother's who were prescribed methadone. What can 'kill' the baby is the withdrawals. Anything mom goes through the baby will go through. Withdrawals from any opiate (including methadone) is very serious. She should not stop taking the methadone, but she should tell her midwife so she can get help from a facility that is licensed to prescribe methadone. If she is not using a doctor she is 'buying it on the street' which is a huge issue. You never know for sure what or how much much you are really buying. People like to water down (the liquid form of methadone) and she and the baby could go into withdrawal. 10mg methadone is such a small amount I wonder if that part of the information is correct? 10mg would not help with back pain, but you could become physically dependent on that amount. I have seen babies born whose mom's were on anything from 30-150mg and they were and are just fine (mentally, emotionally, and physically). If she does not get the methadone prescribed legally before she gives birth she (and the baby) could end up in serious trouble. For one thing the baby may go through neonatal abstinance syndrome (withdrawals) after birth and need medical treatment (the hospitals know how to withdrawal babies comfortably). The other issue is if medical personnel suspect the baby is in withdrawal they will drug test him or her at birth and if they are positive for methadone without the mother being prescribed the methadone by a doctor she will be at risk for having social services step in and remove the baby pending an investigation. If her doctor or midwife and hospital personnel know ahead of time, they will be prepared to give the baby something to keep them comfortable and help with any withdrawal symptoms. Tell her it is best to get help now so she can have a healthy baby."

She should just risk getting in trouble and tell her midwife that she's using methadone (which would be given to her by a physician if she really was in need of it). drugs and drinking during early pregnancy? My appointment isn't until next week. I just found out yesterday morning that I am pregnant. If calculated correctly, I am 9 weeks and 6 days. Up until yesterday I have been drinking, taking lortabs, valiums & methadone, smoked a little pot & ice. I've completely quit everything, but have I harmed my baby?

The alcohol will not effect your baby most likely, the babies that are effected by their mothers drinking have mothers that are major alcoholics and barely have functioning livers (a sip of alcohol effecting a baby is a myth) and as for the drugs if you haven't aborted then you probably haven't done enough to permanently harm the fetus and with completely quitting and good prenatal care your baby should come out fine. But of course you should go and see a doctor to be checked out."

Is it more beneficial to continue or stop prescription drugs during first trimester of pregnancy? I have been or prescribed ritalin 30mgs daily, Clonazepam 3mg daily, and 20mgs of methadone daily for years. I just found out my husband and I are expecting a baby. I have read conflicting letters on the pros and cons of continuing my meds or reducing them. I always thought you should refrain from anything potentially harmful to the fetus. Suffice it to say, I did go through two days of hell coming off so I started weaning now with vicoden. What is the best for baby?

LittleRoo replied: "The BEST thing for baby is to get an IMMEDIATE appt with your doctor or OBGYN....obviously some medications are not good for your baby but neither is immediately discontinuing medications without being under the supervision of your doctor. I was on adderall and paxil when I found out I was pregnant. I called the OBGYN that day and told them the medicine I was on and that I needed an appt right away to discuss this...they will get you in quickly when it comes to medication. I do know, that my doctor advised me to take the medicine following her directions...and NOT to suddenly quit or change dosage without her supervision. She gave me very strict instructions on weaning myself off the medicine by slowly decreasing the dosage. You must, must, must talk to a doctor BEFORE changing your meds...again, call your OBGYN and tell them your situation...I got an appt within 48 hours...they will squeeze you in!"

It is true that what you take, your baby will "take" too. What you and your doctor need to decide is if the risks to the baby outweigh the benefits to you. I have 4 kids and took Prozac with all 4 pregnancies. I know that the meds could have affected the babies, but if I did not take them, I would not have been able to get out of bed. My depression is severe and I had to decide if the risk to my baby from the meds was bigger than the risk from my emotional turmoil. Some doctors believe that a mother's mental and emotional state can affect a fetus just as much as medications can. Bottom line...stay on the meds until you can talk to your OB-and psychiatrist if you have one."

Cindi P replied: "Ritalin is classified as a C...so should not be taken unless benefits far outweigh the risks. Methadone is classified as a B....BUT would be a D if used for long periods of time or high dosages...so would probably be best if you went off it. Clonazepam is also classified as a C."

They actually recommend staying on the methadone until after you deliver because it can be more harmful for the baby to detox inside of you then it is for the baby to detox in the safety of the hospital. I have a friend who just had a baby on March 20th and the baby is still in the hospital detoxing. Besides for being addicted to methadone, the baby is healthy and was born full term and a good weight."

It is indeed a very hard addiction to break. My main concern is not to the developing fetus... However the withdrawl it will have to endure once he/she is born. That will indeed be aweful. However, it is not recommended for her to quit cold turkey because of the withdrawls that the baby will go through in the womb. During the 1st 12 weeks there is too much development going on and it can be very dangerous. After that she needs to have a doctor assist her with the withdrawl process. I myself was a morphine addict (a long time ago) and I can tell you it is an extremely difficult thing to come off of... I've heard methadone is even worse. I do know this much, when I got pregnant I was smoking ciggs about a pack and 1/2 a day. I cut back and finally quit before 16 weeks. It was very hard to do! I just kept thinking of my baby having to go through withdrawl.... It was enough for me to do whatever it took. She may be different. Perhaps taking her to the NICU of your local hospital where she can see babies going through withdrawl will help. If not, perhaps she will consider lowering her dosage. Either way, unfortunately there's not much you can do. I would just encourage her to do the next right thing for her baby. Hopefully all will be well. Very Best Wishes!"

Is this normal for an almost 3.5 year old girl? -Not be able to talk clearly at all and not be able to say many words ( including her name!) -still wears diapers..with no sign of readiness to potty train -she only puts 3 words max. together, 80% of the time you can't understand her Her parents are doing nothing to help her I think something is wrong..From my knowledge her mother smoke, drank, and was on methadone during pregnancy.. could this cause these problems?

Angel Love replied: "All kids develop at their own pace and it is really not fair to compare them. It is her parents and doctors responsibility so do not worry about it. When she gets into school, at the very latest, she will get help."

And to answer your question, yes, these things can cause serious problems. She's lucky the baby survived it all. I would really intervene. Call child protective services if you have to."

It is ridiculous that she is is not potty trained, that shows some sort of neglect ...Her parents are to lazy to teach her. A child needs to be shown and taught and clearly they are not doing that. Lance N replied: "I doubt this is normal for a little girl her age. No mother should ever smoke, drink, or do drugs during pregnancy. It's not very likely the smoking, drinking and the meth may have played a role. I know a 7 year old girl whose speech is no where near perfect but, I can still understand her. No little girl deserves to be neglected or ignored. Try to see if you can have her referred to a speech specialist."

like it like replied: "I see many kids who look like they are older than that wearing diapers. in fact, store has diaper sizes for 4 and 5 year olds! so it seems in demand. Of course thats not a good thing, but if that wasn't so common, wouldn't be. As for talking, my niece learned to talk clearly at 4, and her parents don't do any drugs, and are pretty educated normal people, not alcoholics. I don't know about the family you are talking about, but i WOULDN'T NOT MAKE conclusions just based on what you said. how to you know that she did drugs? Were you there? Or just "your guess"?"

Opal replied: "I am, by far, an advocate of letting each child develop at their own pace. But what you've described above is definitely concerning and may warrant professional medical consultation and treatment. A 3 1/2 year old child should be able to do all of the above. However, since you noted that this isn't your child, are you certain that child does not, in fact, do these things but perhaps not in front of anyone other than her parents? For example, my 3yo daughter can be extremely shy, even in front of extended family who she's seen almost every day of her life! But in private when it's just us at home, she's extremely talkative and lively. Likewise, she didn't potty train until she was 3.5 year old despite the fact that we had begun training her at almost 2yo. She certainly knew how but she apparently wasn't ready. On the same note, she won't say her name when asked to anyone but family. So before thinking that something is wrong with the child you described, you should ensure that your concerns are based on what the child is like even when the child is not with you. The last thing you'd want to do is misdiagnose a child based on inaccurate and/or incomplete information. But based on face value, especially considering the history you mentioned of the mom's substance abuse during pregnancy, the child could be developmentally delayed for reasons beyond just preferring to operate at her own pace. I think the bigger issue then would be how to approach the parents with your concerns without seeming offensive, rude or nosy, especially since you said that her parents are doing nothing to help her. Maybe they feel she doesn't need help or maybe they don't know how to help. There is a big difference and some education via doctors, childcare professionals, trusted mothers and friends could really help them as parents to help their child. When seeking medical attention, ALWAYS get a second and even third opinion. I worked in a medical clinic for 5 years and was shocked and sickened by how many doctors were quick to medicate children for "developmental delays" and "social conditions" instead of being patient enough to find the real root of the problem and create natural and safe treatment plans. Doctors are people too and can make mistakes. So as parents and childcare providers, we must make time to educate ourselves in order to protect ourselves and loved ones from misguided information. In any case, I wish the child the very best."

shir replied: "Call the School Board or even one of the schools in your area and ask them if there is a county educational service center for preschooler's with developmental delays and get the number. Then, encourage the parents to call and set up an appointment to have their child tested (it is free). They will test her motor skills, speech, etc and if found delayed, they can set her up in one of their preschool classes (that are also free) to help her. Another avenue is to encourage the parents to have their daughter checked by her family physician/pediatrician. It is very important to have early intervention for developmental delays and the sooner she gets help the better."
